Registration for the 1999-2000 school year for students new to the Chula Vista Elementary School District will begin Monday, March 15, 1999. Parents may register their children at their neighborhood school during the hours posted by each school. To confirm neighborhood school information, parents may call 425-9600, ext. 1572.
Children who are five years old on or before December 2, 1999 may be enrolled in next year's kindergarten program. The following is necessary for school registration:
Verification of the child's birth date can be completed by presenting an original birth certificate.
Immunizations necessary to enter school are DPT (diphtheria, pertussis, tetanus-a series of four immunizations, with the fourth one given after the second birthday); poliomyelitis (three immunizations, with the third one given after the second birthday); measles, mumps, and rubella (two immunizations given after the child's first birthday prior to entering kindergarten); and three doses of Hepatitis B vaccine.
Proof of residency, such as a current gas and electric bill showing parent name and address, is required for enrollment and again each year thereafter.
A physical examination is not required for children entering kindergarten.
Children entering the first grade who have not attended kindergarten in California must be six years old on or before December 2, 1999. Verification of birth date, immunizations, and residency is required. In addition, all children entering first grade must have a physical examination. Physicals given within 18 months prior to the date the child enters first grade are acceptable. (For example, a child entering first grade on September 7, 1999 must have had a physical examination AFTER March 7, 1998.) A green form entitled "Report of Health Checkup for School Entry" is available at all school offices for the purpose of reporting a physical exam.
